Click in to Edit Your Site and stay on the Build tab

Select the page where you would like this form to be placed

Drag and drop the "Embed Code" widget onto the page wherever you would like the form

Click into the Custom HTML box, highlight the text "click to set custom HTML" and paste in the HTML/CSS form code from BenchmarkONE

Click on the page anywhere outside of this box and the form will load for you
